OSU Wexner Partners With CVS Accountable Care on ACO

Ohio State University Wexner Medical Center Partners with CVS Accountable Care to Create Accountable Care Organization (ACO) to Improve Quality of Care for Medicare Beneficiaries by State of Ohio Providers in Center of Ohio through an emphasis on preventative wellness and proactive treatment of chronic diseases. .

CVS Accountable Care, part of CVS Health, is a value-based risk management and enablement partner that was formed in 2023 as a result of the merger of Signify Health Value-based Care Solutions with CVS Accountable Care Organization. The company enables more than 200 health systems and 60,000 physicians in value-based care contracts nationwide, serving more than 1 million Medicare patients.

The new CVS ACO, a Medicare Shared Savings Program (MSSP) Enhanced Tracking ACO, expands on the work of Ohio State Wexner Medical Center’s previous ACO. CVS ACO will combine resources from both organizations to provide more patients with care coordination and longitudinal care management to address medical and social needs.

The clinical care teams at Ohio State Wexner Medical Center and CVS Accountable Care will initially focus on supporting the transition of care for all ACO beneficiaries following a hospitalization, providing medical and social support services for beneficiaries at higher risk for hospitalization or readmission, and strengthening comprehensive care management. for all OAC patients.

The idea is that patients will benefit from increased points of contact from a member of the clinical care team, including education about complex medical conditions, assistance in scheduling follow-up visits, and access to community resources.
